By accessing our website, scheduling an appointment, purchasing a service, or using our services, you acknowledge and agree to these Terms & Conditions.

Services

The L.A.B. Testing & Compliance Solutions provides testing, screening, specimen collection, fingerprinting, background screening, employer testing programs, mobile collection services, notary services, and related professional services.

Certain specimens, tests, screenings, background checks, DNA analyses, confirmations, and other services may be processed or performed by independent laboratories, government agencies, or third-party service providers. The L.A.B. may serve as the collection or coordination provider and does not represent that all laboratory analysis is performed directly by The L.A.B.

Appointments & Identification

Appointments are subject to availability. Clients may be required to present a valid government-issued photo ID and any documentation supplied by an employer, agency, attorney, court, school, program, or other requesting organization.

Clients are responsible for providing accurate information when scheduling and completing services.

Payments

Payment may be required when booking or before services are performed unless other arrangements have been approved.

Additional charges may apply for laboratory confirmation, specialized testing, third-party processing, mobile service, after-hours service, travel, or other services outside the original appointment.

Cancellations, Rescheduling & No-Shows

Appointments may be canceled or rescheduled according to the cancellation period displayed during booking.

Late cancellations and no-shows may be non-refundable. Clients arriving late may be required to reschedule when there is insufficient time to properly complete the scheduled service.

Testing & Results

Processing times and result availability vary depending on the service performed.

Rapid screening results are preliminary when applicable. Non-negative, inconclusive, invalid, or otherwise qualifying results may require additional laboratory testing or confirmation.

When specimens or information are submitted to an independent laboratory, government agency, background-screening provider, or other third party, processing times are controlled by that provider. The L.A.B. does not guarantee third-party turnaround times.

DOT & Regulated Testing

Services associated with DOT-regulated or other regulated testing will be performed or coordinated according to the requirements applicable to the requested service.

Where laboratory analysis, Medical Review Officer (MRO) services, or other regulated functions must be performed by an authorized third party, The L.A.B. will coordinate those services as applicable rather than representing them as services performed directly by The L.A.B.

DNA Testing

The L.A.B. may provide specimen collection and coordination for DNA and relationship testing. DNA analysis may be performed by an independent laboratory.

Requirements differ between legal and non-legal testing. Clients are responsible for selecting the appropriate service for their intended purpose.

Background Screening & Fingerprinting

Background screening and fingerprinting results may depend on information maintained by government agencies, databases, or third-party providers.

The L.A.B. does not control the contents of those records or guarantee that an outside database contains complete or error-free information.

Mobile & On-Site Services

Mobile and on-site services are subject to availability, location, staffing, service requirements, and applicable travel or after-hours fees.

Additional arrangements may be required for employer groups, post-accident collections, after-hours collections, or other specialized services.

Confidentiality & Privacy

The L.A.B. is committed to handling client information professionally and confidentially. Information may be shared with laboratories, employers, agencies, requesting parties, or service providers when authorized by the client or otherwise permitted or required for the requested service or by law.
